Some of our guidelines are as follows:

* Delaware resident
* Age between birth and 18 years of age * US Birth Certificate

Must provide the following:

* Photo release form(a childs full name and address is never release to the public)
* A picture of child
* A Summary of the Medical Condition
* If at all possible attendance at the event that provided the funding (in some cases it may not be possible)

Notation:

The funding is provided directly to the supplier.

*Delaware Resident, as qualified must reside in the state of Delaware 365 continuous days [1 year]
